We provide Community First Services and Supports (CFSS) under the agency model, together with an integrated home care license carrying the Home and Community-Based Services (HCBS) designation. In practice that covers personal care and assistance, homemaking and chore services, community access and transportation, respite and family support, health and wellness support, and coordinated HCBS and home care.
Older adults, individuals with disabilities, individuals with chronic conditions, and anyone who needs support to live independently. We support people of all ages who want to stay safely in their own home and community.
