Health and wellness support is the everyday, non-clinical help that keeps you feeling well at home: eating properly, keeping moving, staying on top of routines, and having someone around who notices when something is not quite right.
It is not medical treatment and it does not replace your doctor or your nurse. It is the ordinary scaffolding around your health – the meals, the walks, the reminders, the regular company – that makes the medical side work better and makes the days feel steadier.
Most of what keeps people well at home is unglamorous and repeatable. That is exactly what this service is built to provide.
Wellbeing at home is mostly rhythm. Getting up at a reasonable hour, eating something proper, moving a little, having somebody to talk to, going to bed at a sensible time. When those slip, everything else gets harder.
A caregiver who comes regularly holds that rhythm in place. Not by nagging, and not by taking over, but by being a dependable part of the week – which turns out to be one of the most effective things anyone can do for how a person feels day to day.


One of the quiet advantages of regular home support is continuity. A caregiver who sees you every week knows what your normal looks like, so they notice when it shifts: less appetite, more breathlessness on the stairs, a new unsteadiness, a bruise, a flatness that was not there before.
None of that is a diagnosis, and we would never present it as one. But raising it promptly with you and, where you want, with your family or your care team, is often what turns a developing problem into a manageable one.
This support sits alongside whatever medical care you already have. Your doctors and nurses lead on treatment; we help you follow through on the everyday parts at home – the appointment you need to get to, the meals that fit the advice you were given, the medication schedule that is easier to keep with a prompt.
We will always tell you what we have observed and what we have passed on. Nothing about your care should be happening behind your back.
Support usually begins with a conversation. An assessment determines what you are eligible for, and a support plan is agreed with you and your lead agency or case manager. We then agree the visits and the routines that make the most difference to how you feel.
Older adults, individuals with disabilities, individuals with chronic conditions, and anyone who needs support to live independently. Health and wellness support is one of the services covered by our integrated home care license carrying the Home and Community-Based Services (HCBS) designation, across 11 Minnesota counties.
