Fulfilledlyf Care and Community Services is a Personal Care Assistance (PCA) services provider agency enrolled with the Minnesota Health Care Program (MHCP). The agency offers exceptional person-centered services to people with intellectual disabilities and older adults in the most integrated Community settings. We believe individuals with intellectual and Developmental Disabilities as well as those over age 65 deserve to live a remarkable life regardless of their background, social, economic, or cultural history, and are supported by trained, dedicated and compassionate staff to develop independent living skills and opportunities for personal growth.

Fulfilledlyf Care and Community Services currently serve the Twin-Cities including Minneapolis, St. Paul, Hennepin County, Anoka County, Ramsey County, etc.

We work with all Community Service Boards (CSB) within our jurisdiction including St. Paul, Minneapolis, Hennepin, Anoka, Ramsey, and Washington.
